Needs work – CFBuzz Review

The date fields are screwy. I select June 6 & the App adds a date of Sept 6. I selected 2/20/2017 for a date & the app displayed 20/20/217 for a date. In the app. I was going to offer feedback but app kept taking me to a patient survey. I had to go to Ituns App Store to provide feedback.
