Stuck at assessment for vaccine registration

My wife and kids have been trying to register for their vaccination but the app is stuck at the assessment phase. It kept asking the same questions on update registration details, update home address and update pregnancy status. Every time they answered the questions, the same questions repeated.

It blocked USA’s IP Address

After recent updates, the app and website are no longer can be accessed from USA, it looks like it blocked USA’s IP address. If using a VPN that has Malaysia IP address then only can access but then VPN is a paid service. Please have this problem fixed ASAP

I forgot my password and when I tried to reset it, the button doesn’t direct me anywhere else or send me an email. I emailed the help desk with no response.
This app doesn’t support IOS 15

My digital vaccine record was missing after being updated to ios15. The app keep crashing. Please help!
