Really awful. Searching for providers is a waste of time. No matter what criteria you ask for, it doesn’t matter because it’s going to list everyone on earth. It also doesn’t seem like this company has any intention of putting any effort into improving this app.

Just received policy renewal cards in the mail on 11/1/14, with a big emphasis on the stock paper that you detach the cards from, about a mobile app that shows your ID card. I thought, how convenient because I only got 1 set of dental/vision cards, so my wife can take the cards and I'll download the app. Installed the app, tried to open it, it hangs on a blank white screen for 1-2 seconds, and then just crashes and closes back to my iPhone home screen. Tried it multiple times to no avail. Pretty useless and what a waste of ink promoting an app on all their renewal card notices that doesn't even work. Disappointing.

Won't let me select anything other than 5 miles and general dentistry. Then responds 'no providers'.

This app is very buggy on iOS 9.2. Repeatedly returns you to the main screen when searching for a provider without completing the search. Also repeatedly clears all entries when trying to set up your card for the first time. Screen can only be viewed horizontally.
