Need help - my log in info doesn’t work

After downloading the app and trying to setup my account, I’m not able to log into the app. Did the “forgot password” and still doesn’t work. Please need your help and hope my humble request will not be omitted.
Not Significantly Better Than a Regular Online Form

You can easily replicate this app with a MSoft of Google Form. I regret spending $20 on this, especially since it’s not nearly as customizable as any of the aforementioned options. The key problem is that they only allow for 5 “domains” when most states have more than 5 standards. I need to be able to address 9 standards during a walkthrough. I’d like my $20 back for sure.
