Paid $2.99, but ads have reappeared – Utility Spreadsheet Review

I paid $2.99 to remove the ads, but after a few days, they returned. I clicke "Report a Problem" and got nowhere—an email message opened, but there was no address in the "To" field, so I couldn't send it.
