I switched from a cumbersome paper-and-pen form to a form developed with Device Magic’s form development software, used on my iPhone. You do need to set up an account in order to use Device Magic's online development software but the account is free for use with one device and you can receive the form in your email, GoogleDocs, etc. Some options are not free, but these two are, and are the ones I need. The form is entirely setup "from scratch" as it's entirely customized to your needs (there are no "stock" forms), but it was easy to do with their new online development software. The time spent was well worth it! You can create fields for a customer signature, GPS location, a photo of the customer's equipment... all fields can be made optional or required or dependent on other answers... Great level of customization! The Device Magic form is much more consistent than our old method and it’s impossible to accidentally omit critical information on the form. And it never fails to impress clients, either; they seem to especially like the signature field feature. Their expressions are usually priceless when I ask them to "sign" the form after they read my disclaimer (also in the form). During development of the form I had some questions and needed assistance. I contacted Device Magic on several occasions and I’m very happy with the support I received from them. The response time was very good and the level of knowledge was excellent! Simple, powerful and flexible. This app allows us to capture data from multiple users in the field. Data capture is sent directly to Google Doc spreadsheet as well as email address if we choose. We have been empowered to read metrics in real time rather than recording data on written carbon paper. The guys who do support for this App are awesome!

This product does a good job at what it is designed for. The company continues to actively support their product and is very timely with fixing most issues. I was able to take forms I had created in another defunct form entry program and rebuild them to Device Magics platform quickly and easily. The fact that they allow import of data using csv helping speed the process up!

Needs a pricing tier between free and professional

I really like this app. I needed something that could collect data offline and then save it in the cloud one my device was back online. I have one very specific use case for this and two forms I use. It works great for this. Easy form designer on their website, easy data entry in the app. The only thing I need beyond what the free version does is to be able to store the data in Google Drive. To get that capability I need to upgrade to the professional version. For $24.99 per month, I'm not about to do that to support my one device which I use to collect about 3 data points (form fills) per week. So, I'd like to see a personal use edition that gives at least the capability to write to Drive, DropBox, and/or Box for us low end users. Also, the app does not allow negative numbers for integer and decimal fields. I need them for my use case and I'm sure many others do too.

This app is really good you can creat your own template/file and customize it but you must go on there site first to do it. I use this for my security job is good for it you can write a lot and upload images or sing and send it to your email. There is so much you can do with this app
