I have question what’s is the look up or combination in the app

And I’m not sure how to navigate through it. I have an iPhone X. I got the companion but what is the log in page mean. There are no instructions on this. Please help. I don’t see any instructions on the actual app either. ?-I really want to play. I am so confused. Please help. Deme
